Cancellations

If you are unable to attend your scheduled appointment, please cancel at least 24 hours in advance of your appointment time whenever possible as a courtesy to your fellow patients and our staff.

Flu Shots and COVID-19 Vaccines

In-Person Visit Guidelines

For the safety, security and health of our patients and staff, we ask for your cooperation with the following guidelines when coming to our clinics. Thank you.

• No weapons
• No smoking
• No recording or videotaping
• No pets, except for registered service animals
• Masking policy (Effective 5/22/2023)
  • Patients with any respiratory symptoms (stuffy nose, cough, sore throat) are required to mask.
  • Masking onsite is optional for patients, visitors, and staff, and we support everyone’s individual choice. Masks are available at clinic entrances and upon request.
  • If requested by the patient, staff will mask when providing care.
  • Staff must wear N95/PPE when providing care to patients with respiratory illness, if COVID-19 has not been ruled out with a negative test.
• Patients and visitors with known COVID-19 should not come into the clinics.
