Login to Your MyChart Portal

MyChart Patient Portal

MyChart is the best way to manage your health care. Log in to your patient portal to communicate with your health care team, view test results and more.

If you have a MyChart username and password, please log in using the box on this page.

Sign Up for MyChart

If you do NOT have a username and password, follow these instructions to sign up for your MyChart patient portal:

  • DO NOT click the “Sign up” button in the login box, instead start the process by emailing MyChart_Help@packardhealth.org with the following information:
    • Your full name
    • Date of birth
    • Phone number
    • Whether you want to activate via email or text
    • The name of your Packard Health primary care provider
  • Once we receive your email our staff will activate your account as soon as possible. You will receive a text or email message within 3–5 business days with instructions to complete your activation.
  • You may also activate your MyChart portal at your next office visit.
  • If you urgently need immediate access please call us at (734) 971-1073.

What can you do in MyChart?

  • Communicate with your doctor. Get answers to your medical questions from the comfort of your own home.
  • Access your test results. No more waiting for a phone call or letter—view your results and your doctor’s comments within days.
  • Request prescription refills. Send a refill request for any of your refillable medications. Coming soon!
  • Manage your appointments. Schedule your next appointment, or view details of your past and upcoming appointments. Coming soon!
  • View your billing account. See account detail, view statements, sign up for paperless billing and pay your bill

  • MyChart Support Click here to submit a MyChart support request. Technical support for MyChart is provided by University of Michigan Health-West.

Note: Packard Health’s Epic software is hosted by University of Michigan Health-West. Packard Health remains a separate entity, and is not a part of UM Health-West or Michigan Medicine. Your medical record will continue to be held by Packard Health, but occasionally you may receive messages or technical support from UM Health-West regarding your MyChart account.
